Does the iMac have fans

My iMac just today started making a sound that sounds like a fan running and the back of the unit seems hot to me

Yes. iMacs have fans. The top left hand corner can get warm, especially if you are playing games for some time.

The 2006 thru 2011 iMac models have three fans. (HDD, CPU and Optical Drive)

The new thin 2012 models only have one.

If it is an older Intel imac model, when was the last time that the Air Intake (the bottom intake grill work and small intake behind the stand) checked and cleaned.

Use iStat Pro or Temperature Monitor if you want to check your Temps and Fan Speeds.
