Was this phone hacked to carrier unlock or jailbreak?
No, it is not.
Its a unlocked phone purchased in apple store.
I have reset the phone as below.
1. reset network settings. - No result.
2. reset all settings - No result
3. Erase all content and settings - No result
In Settings - General - About - Wifi Address is Showing N/A.
I wonder if your Wifi MAC begins with F0:DC:E2.
If it's N/A, you can check Bluetooth's MAC, since normally the first 5 hex-digits are identical.
Bluetooth MAC showing as - A8:FA:D8:0B:D5:0F
