No app, it's a feature of iOS.

If you are not getting a window in the middle of the screen displaying caller info, there is something wrong.

Do you use a gsm or CDMA network?

Does your carrier support caller ID?

The app is called "Phone" and it is built in to the iPhone. If your carrier provides caller ID then the Phone app will display the name of every caller who is in your contact list. If the carrier does not provide caller ID or you have not chosen to enable it then there is no way that ANY phone (not even a Nokia) can identify the caller.

do you mean speak the name out loud so you can hear it?

You mean 'speak' the name? The only way to do that would be to set up custom ringtones with a recorded voice for each person in your contact list.

How would the phone know the name of the caller without caller ID?
