checkered pattern and blue screen

this is the mac im running, it just sarted having these little lines or diamonds shapes in a checkered or diagonal pattern across the screen, when i reboot up it stays on a blue screen.

I already tried starting with r,p r command and option, started in safe mode, and held command and s and check /sbin/fsck -yf and it came back ok.

thought it was working after that and it happened again, im on it right now but i'm waiting for it to happen again. can you help?
