Activation problems are almost caused because the phone was hacked to unlock it from the original carrier and then updated or restored.
It may also occur if you are attempting to activate a phone without a SIM.
A SIM must be present to activate an iPhone. If the phone is carrier locked, it must be from the correct carrier. It does not have to be tied to an active line of service.
