Call Apple Support and arrange service. Apple will send you a box with a pre-addressed shipping label. Do not attempt to send your iPhone to Apple without setting up a repair, or Apple will just reject the shipment.

Sorry, this appears to be something new (or at least is new to me). Previously Apple provided a box, but I see now (going through a fake support request) that you can indeed take the iPhone to a UPS Store. I would presume that the UPS Store will have all the appropriate information, since Apple doesn't give in as part of the online service request procedure.
