How do I get shattered crystal replaced on iPhone5
My iPhone 5 fell out of my shirt pocket onto the cement floor and the glass face shattered. Is it replaceable? Who replaces it?

You can buy an out of warranty replacement from Apple ( damage is not covered by warranty) for $229.
You can look for a third party repair provider.
