I know there is a 'monthly' repeat but that isn't four weeks. I, and probably hundreds of other people, need a four week reminder when ordering medicines.

For some reason Reminders does not use the same code that Calendar uses for repeats. A real mixup imo.
If you set this up in Calendar you can create custom repeats
You should send feedback to Apple see https://www.apple.com/feedback/ There is no category for Reminders so I would just use Calendar and note that the feedback is for Reminders.
Thanks a lot Frank, I have done what you syggested re contacting Apple feedback. My problem is Reminders on my iPhone. The alarm sounds to remind me but this doesn't work in Calendar.and it's possible I may miss it. Thanks again for you help
If you set this up in Calendar as a repeating event you can have it appear on all you devices (Mac and iPhone) just as the Reminders do. And the Calendar even can be set to appear in the Notifications.
There is really not much difference between Reminders and Calendar events.

The problem is that I don't use a Mac computer, just the iPod, iPad and iPhone and the custom repeat setting is only available on a Mac. I can't see that it would be very difficult to include ia 4 week repeat on the next OS update. I'm not holding my breath though!!

Probably not the solution that you're looking for, but I do also have a need to be reminded every four weeks, not every month. I want to be reminded to pay rent/utilities on the day that I get paid (which is every other week, not twice a month).
Here's what I did (and I realize that this solution may not be available to you): I asked Siri to remind me to pay rent every four weeks. It sets up the custom reminder for four weeks from today, and I then I went in and changed the date to the day that I wanted.
Again, not a perfect solution, it might not be available to you, and you're absolutely right that it should be an option built in to the reminders app, but it's a workaround until the feature is added.
