I have 654 contacts on All Contacts, 591 on All on My Mac and 610 on All iCloud. How do I get the full 654 contacts on all of them? Obviously it's the All iCloud that worries me.

All Contacts is simply a view of the iCloud an the On My Mac contacts, it is not different from them, it is a comosite view of the other 2.
Identify which contacts are missing from iCloud and then move or copy them from On My Mac to iCloud.

I understand and .... how do I identify the 44 missing contacts from among the 654? That's my question really. Is there an elegant (or wholesale) way to do it?

I would do it the hard way, look and move.
But if you are looking for wholesale, move all contacts from On My Mac to iCloud, then use the 'Look for Duplicates' command in Contacts.
Back up both accounts before doing this.
Actually, Applecare helped me do it elegantly which essentially consists of merging All on my Mac with All iCloud and ask the system to delete duplicates (this on takes a few iterations).
Ther's a bit more to it than I'm telling you, but it all takes about 2 minutes.
