Some of my smart playlists won't play songs unless I specifically
select a song, play it, specifically select the next song and play it.
It may have to do with live updating, and this has been an issue for
at least a couple of years with me.
Is this a setting issue? Or is it a bug?

Make sure all the checkboxes are checked. iTunes will not automatically advance to an unchecked song.
If you don't see the checkboxes, first go to Edit > Preferences > General > Show list checkboxes.
They are checked.
The playlist I use the most has:
Match all of the following rules:
Plays is 0
Kind is not PDF Document
Kind does not contain book
Genre does not contain Audiobooks
Live updating.
This picks up podcasts that I haven't looked at.
