Can anyone tell me how to access the sync apps button please? I used to be able to select it but it is no longer visible.

In iTunes go to Edit>Preferences>General and make sure the "Apps" selection button is ticked.

Do you have Restrictions enabled on your iPhone?
If so, so you have installing apps turned on as a restriction?

Restrictions being off means there are no restrictions.
Is there an Install selection next to an app under the Apps tab for your iPhone sync preferences with iTunes that is not installed on your iPhone?

No - the only option next to all the Apps is "remove".
That means all apps in your iTunes library are installed on your iPhone.
When I update an App on my laptop, but not on my phone, the option to update or sync Apps still is not available. If I download an update on my phone I use "transfer purchase" to update iTunes on the laptop, but nothing seems to work the other way.
If you update an app from your computer, the update will be sync'd to your iPhone on the next connection, though you may need to initiate the sync by clicking the sync button while connected to iTunes.
Not needed if an app that is already installed on your iPhone is updated with iTunes since the update will be installed on your iPhone automatically with the next sync after the app update is downloaded with iTunes on your computer . For the most part I download app updates with iTunes on my computer and the app updates are installed on my iPhone and iPad automatically for all apps in my iTunes library that are currently installed on my iPhone and iPad.
The new iTunes 11.0.3 has moved the button to the top when you select the Apps (left panel).

You have to connect and select iPhone in computer iTunes Sidebar.
If you don't see the Sidebar, toggle it on in iTunes menu View > Show Sidebar.
