FaceTime shortcut disappeared

Question

My FT shortcut was accidentally deleted from my iPhone, how do I get it back?

iPhone 5, iOS 6.1.4

There is no FaceTime icon on the iPhone, instead it is accessed from within the contacts app.

There is not FT icon on the iPhone's home screen. Access FT through Contacts.
