broken toolbar icons in finder, safari, mail

hi there,

my problem is broken toolbar icons in finder, safari, and mail, search bar

it happens suddenly, i don't use thirdparty addon for the appearance.

recovering with clean install is my last option.

i have tried with logging in-out, reinstall update, deleting finder .plist, still no result.

tried to run 10.7.5 combo update but it couldn't run.

can somebody give me a clue to fix it?thanks.
