iTunes audio output stops but playback continues?

This is seriously weird... iTunes 11.0.3, OS10.8.3 playing back audio & totally randomly the audio output will stop but the progress bar of playback in iTunes shows the song to still be playing.

Clicking pause & then play will bring the audio back immediately.

This will happen to whatever source the audio is playing through so tonight it was playing on my computer & out through the AppleTV when the audio stopped a few times. Then it didn't happen again for the rest of the evening.
