On my IPhone 5 Settings>General>Restrictions = Off and I can't make any changes and turn camera app to on, what am I missing?

If the camera is disabled without you having Restrictions enabled, then do you have an Exchange email account set up on your phone ? If you do then it's possible that the account administrator is disabling the camera.
