    Then you will have to force the phone into recovery mode and restore it as a new device using iTunes.

     

    You or someone with physical access to the phone had to have set the restrictions passcode. It can't set itself.

    If your iPhone is connected to a company Exchange server or otherwise managed by them, they can set and enforce restrictions and set a passcode to keep employees from changing them. If this might apply to you, talk to your company's IT department.
