iTunes keeps opening as if it is a fresh install (no settings remembered)

Perhaps someone can assist me. After updating to 11.0.3, I started having an issue where iTunes opens as a fresh installation (prompts with the user agreement, doesn't save view library settings, makes me re-sign in with my apple id) each time I open the program. I tried updating to 11.0.4 and performing a fresh (clean) installation but the issus persists. It still sees my library but I have to sign in to the store, change out of album view, set up side bar, etc. each time.

Worthwhile information: Windows 7 64-bit, iTunes media (songs, tv shows, apps) stored on a separate internal disk drive.

I would appreciate any thoughts/suggestions on the matter.
