Why Are Surnames Continually Added Back to Contact Entries After I Delete the Surname?

I have quite a few contacts with just their first name.


The Contacts app on my MacBook Pro continually adds last names -- if I had it in there at one time and since removed it -- back to the contact. Then this syncs over to my iPhone and iPads.


Why does Contacts do this and how can I stop it permanently?

MacBook Pro, OS X Mountain Lion (10.8.4), iPhone 5 iPad iPad mini

If contacts are reverting back, the next place I would try to fix is at iCloud.com, from there you can edit from the source of the contacts. I have also provided and article that will assist with troubleshooting contacts across multiple devices.
