    The same issue, there is no button to request an agreement.

    But accoring to JavaScript inside Contracts page, those buttons should exists:

    <script type="text/javascript">

      var set_dropdown_actions = function(){
      var set_request_button_state = function(select_field){
        var image = select_field.up('div.ajaxListRowDiv').select('img.ajaxListItemRowImage')[0],
            val_is_empty = isNaN(select_field.value * 1);
        if(val_is_empty === true){
          image.src = image.src.split("request-blue-btn.png").join("request-disabled-blue-btn.png");
          // request-disabled-blue-btn.png
          image.src = image.src.split("request-disabled-blue-btn.png").join("request-blue-btn.png");

    I had problems in the past that were resolved by changing browser or clearing cache... Apple has a weird way of dealing with this...

    I'd tested it in Safari, Chrome and Firefox - no button displayed.

    Sometimes I think you have to email apple support, possibly multiple times. I had to do this, and after getting a canned, generic response - my button appeared as well as some other stuff I needed to approve.

    You have the answer on 2 page. Just click Request amendement in your "Contracts, Tax, and Banking" and agree with new agreement.

    Please find me "Request amendement" on the screenshot.

    Were you able to resolve this. Stuck with this issue and couldn't do anything. Thanks in advance.
