My FaceTime icon has disappeared from phone screen?

FaceTime has never been a separate app on an iPhone. You initiate a FaceTime call through the Phone or Contacts app.

That which does not exist can not disappear. Unless you came here from the future, where it will reportedly actually be a separate home screen icon beginning with iOS 7, then you are mistaken.
Ok, thanks. How do I know who is on face time when I look in contacts
look at a contact in your contacts app. there is a button that says 'facetime.'
