Airport Extreme Extended Setup

Does the airport extreme have to be physically connected to an airport base station to extend the wireless signal?

No, it may extend via wifi.

No, you can extend wireless (more normally called wireless repeater) or you can use cabled connection and setup roaming network. You pay a cost in speed repeating wireless.. usually 50% slower.
