I have Name and pawword chosen in Users & Groups, but the login screen is still a list of users. Is there something I can do to force the OS to display Name and password like I want?

If you just want a username box and password box, then go to the Users & Groups system preferences, and click the "Login Options" section underneath the accounts list. Then click the radio button for "Display login window as" to set it to "Name and password."
Now when a user logs in, he or she will have to type the username followed by the password, instead of selecting a username and the only typing the password.
Did you even read my description? As I've already indicated, Name and password is what I have selected in System Preferences -> Users & Groups -> Login Options -> Display login window as. That's the problem I'm having: it still shows a list of users at login. 😐
Go to the Macintosh HD > Library > Preferences folder and remove the file called "com.apple.loginwindow.plist" and then try the setting again.
1) Reboot
2) Try making a new test admin account and see if the problem still occurs.
