My OS is Mountain Lion 10.8.4. When I press Command-T to open a blank document in Safari; it says the document is "Untitled Document but it keep loading and unknown image, and I can't get rid of it. I've tried finding the file in my computer to delete without any luck. I've scanned with Clamware and everything is apparently fine.
Anyone?

Reset Safari from the Safari menu.
Check all the boxes.
Lol, I meant to say I press Command -T to open a blank page. Basically, search history page has been disabled and replaced with an annoying jpeg instead. In other words, my thumbnail preview page has been replaced with something I don't recognize and don't want.
Tried that. I also deleted all the files in my webpage preview folder.
From the Safari menu bar, select
Safari ▹ Preferences ▹ Extensions
Turn all extensions OFF and test. If the problem is resolved, turn extensions back ON and then disable them one or a few at a time until you find the culprit.
I actually don't have any extentions so it's coming from somewhere else. Also, I just reformatted my drive so everything is clean. I guess I should get some extensions after I figure this one out so it doesn't happen again.
K figured it out. It was hiding in my homepage.... thanks for the info.
