You have a Late 2011 MacBook Pro, so use OS X Recovery to do this > http://support.apple.com/kb/HT4718 Follow these steps:

1. Make a backup of your files with Time Machine, Carbon Copy Cloner or Disk Utility's Restore feature. Time Machine is sufficient.

2. Hold Command, Option (Alt) and R keys while your Mac is starting and open Disk Utility. You need to be connected to a network.

3. Choose Macintosh HD in the Disk Utility sidebar, go to Erase tab and erase the drive.

4. Close Disk Utility and reinstall OS X. It will be downloaded from the Mac App Store.

5. If you are going to sell the Mac, turn it off after finishing the installation and contact with Apple in order to deauthorize your Mac and OS X, iMovie, iPhoto and GarageBand > http://support.apple.com/kb/HE57

If you aren't going to sell your Mac and you just wanted to restore it to factory settings, remember to install iMovie, iPhoto and GarageBand from the Mac App Store after setting up your MacBook Pro
