Device not showing up in Find My iPhone

My phone and wallet went missing last night. If my iPhone no longer shows up under My Devices, does that mean it has been erased or restored to default settings?

Perhaps, but it also may simply be turned off, out of battery or just in a location with no signal.

If the device was off / offline, it should still be in the list.

How can the device be removed from the list by someone else? And is there a way to still find it?

It seems that a user (or thief) can remove the device from the list in Find My iPhone without the AppleID. One proven method is upgrade to iOS7, there might be more.
