You must first enable it on your account with your carrier. See: http://support.apple.com/kb/HT4517 for more information.
You need to configure this feature by contacting your carrier.
Does you cellular provider offer a tethering plan (required for HotSpot)?
Have you subscribed to a tethering plan?
I checked with my provider, they provide teathering facility. Its that in my settings, it doesnt show personal hotspot
And have you subscribed to a cellular data plan that includes tethering?
Yes
Have you gone to Settings>General>Cellular and turned Personal Hotspot to "On"?
I don't think the OP can see that option.

I was guessing they coudn't see it on the main settings page. My experience is that it doesn't show up there until you've enabled it under Cellular. Apparently, I was correct. This time. 😉

Glad it helped.
Interesting, I haven't used Personal Hotspot for around a year, but seem to recall that as soon as it was set up with AT&T, it appeared in the main menu, right above Notifications settings.
wjosten, I believe it was, said he'd never seen it work this way either. However, I've seen this over and over again. Perhaps it's carrier related? I'm dealing mostly with Verizon phones. Until you go in and enable it under Cellular, there are only three items in the top block: Airplane Mode, WiFi and Bluetooth.
.
Perhaps it's carrier related?
That's what I'm thinking, too.
In any case, glad the OP got it set up.
