If I don't have my earbuds I can't walk down the street while talking on Facetime. Well I can but it's really annoying. I'd like to be able to hold the phone to my ear like on a regular call. My friends don't care about seeing me.

I do not think there's anyway to disable the speaker for Facetime, other than using headphones.
I don't understand the problem... if your friends don't care about seeing you, then why are you using Facetime in the first place?
Skype will do what you want.
Oh, yeah. That was a good question. Comes in real handy for avoiding international cellular charges when I'm in the US and the person on the other end is sitting at his mac in another country.
It also has the advantage of working with people who have Android, Blackberry and Windows phones and Windows computers. Of course, it's not as nicely integrated as FaceTime but one can't have everything, I guess.

Other than Skype, you'll just have to wait for iOS 7 to be released and use it's audio only FaceTime feature!
