I am setting up back to my Mac with my Apple Extreme behind a Mikrotik; however, I get this (see attached pic). So, Should I leave the public UDP port blank?

Your screen shot was removed possibly because it showed personal information. Try taking another screen shot and use Preview to cover up any personal information.

Thank you for responding. I gave that pic for an example only; so, the "personal info" does not exist. I have a gateway cable modem Cisco DPQ 3212 which issues only an IP address to a Mikrotik. The Mikrotik does the DHCP-NAT and the Apple Extreme (AE) is Just in the bridge mode.
However, to use the Back To My Mac feature of the AE, DHCP-NAT needs to be enabled which produces a double NAT. I have my Apple account set up also as well as looked at those two documents already (in the link you provided).
So, the only way to show the problem was to provide that picture for an example. Could you help with a solution? The AE has four devices attached or connected to it (AppleTV, Emac, Blue-Ray player and an Android Smart TV).
The Mikrotik does the DHCP-NAT and the Apple Extreme (AE) is Just in the bridge mode.
Put the Mikrotik device in bridge mode and configure the AEBS as your NAT router. Then follow these instructions:
