How do you get rid of the iOS diagnostics tab?

How do you get rid of the iOS diagnostics tab when multitasking between tabs/apps? Even when only one app is open, the iOS diagnostic screen is there when using the multitasking gesture. When I enter in the ticket number sent to my email, it does not recognize it. I've already tried syncing with iTunes, turning the ipad off and on, hard reset, and resetting to factory settings. It does not show as an app or anything for me to close.

the solution worked for me the first time i turned auto send off. i no longer get the bad ticket form (ALWAYS tells me the ticket is bad even though it is the ticket number that was just emailed to me !! does it have something to do with i am no longer in the 90 day free ??? a diagnostic should still be free if my device is bad i want to know why !).

iPad 2 iOS 7.0.
