Sounds as if your phone was hacked to activate and now it is locked back to the original carrier it was locked to. Are you using a supported provider? Where did you purchase the phone?

However, like Chris said, if it was jailbroken, it's locked back to the original carrier.  If that's the case, call that carrier and request an authorized unlock.
