Forgotten Restrictions Passcode Help

You will need to restore your device as New to remove a Restrictions passcode. Go through the normal process to restore your device, but when you see the options to restore as New or from a backup, be sure to choose New.

Also, see iTunes- Restoring iOS software.

No, if it's disabled then it's too late to take another backup and/or copy your content off it - you will need to connect it to your computer's iTunes and reset it back to factory defaults (you may need to put the phone into recovery mode in order to be able to reset it : http://support.apple.com/kb/ht1808), and you can then restore your last backup onto it, or just resync your content.
