How do you rescue a password if your rescue e-mail is no longer in operation and you can't remember your security answers

I have lost the password to my icloud account. I cannot remember my security questions and the alternative rescue email address I gave is no longer in operation - the ISP went bust. What can I do?

Apple ID Security -

Telephone Apple Care for your country and ask for the Account Security Team, then request help resetting the questions, etc.
