I verified the headset jack is fully inserted / flush, reset Nano, updated software....same issue reoccurs. Is there a known hardware issue with the headphone jack contacts wearing out? Any idea what Apple will charge to repair?

Hi, i have the same problem, i take mi iphone and put the videocamera and turn on the light i have garbage in the plug, i retired it and it was all ok, i'm gonna publish a video on youtube
I used a toothpick (maybe not the wisest thing to do, but desperate is desperate) and gently moved it around inside the earphone jack spot. I pulled out a tiny bit of fuzz, so I repeated several times and pulled out an impressive amount of fuzz. Now the ipod is working fine.
