You should be able to create it via a computer's iTunes e.g. if you want to create it without giving credit card details : http://support.apple.com/kb/HT2534
- if you are currently logged into an account then log out of it (Store > Sign Out menu option)
- find a free app in the store
- click on its free price
- click on 'create Apple id' on the popup
- agree to the terms and conditions and fill in their details
Those screens in iTunes should pick up the language that is set on your computer (they do on my Mac), the only screen that I get in Bulgarian if I set my country to Bulgaria is the terms and conditions page.
If you want to read the terms in English then if you initially select the UK (you should have a flag at the bottom right of the store's pages) and then do the above steps - when you've filled in your details and are on the payments screen, at the top of that you should then be able to change the country to Bulgaria :
You will then be taken back to the terms screen which you can agree to and 'continue' and the personal details screen should be pre-populated with what you've just entered and you can then go to the payments screen and enter their Bulgarian address.
