USB troubleshooting
If you are using USB to connect your computer to Personal Hotspot and you are unable to get an Internet connection, check your computer's network settings. You may need to adjust your computer's network settings to disconnect and reconnect the USB Ethernet interface.
Mac OS X
Open System Preferences > Network. Locate and select the Ethernet adapter listed, and then click the gear icon and choose Make Service Inactive. Then choose Make Service Active:

Nov 7, 2013 4:56 PM in response to matt00by David Sprung,I have found that the solution is to reset the networks in the Settings menu of the iPhone. Go to Settings/General/Reset (at the bottom of the menu) and select Reset Network Settings. This does wipe out your current network settings, but they are readily reconfigured if you remember the passwords. Then when you enable the personal hotspot, your iPad or whatever will connect to the personal hotspot if you enter the appropriate password.
