Recurring kernel panic 10.7.5 iMac

I've been encountering irregular, but consistent kernel panics for the past three months. I'm submitting reports, but there don't seem to be any updates in the channel that have affected the situation. I'd like to get to the bottom of this. The machine was completely reinstalled from media one month ago, and the situation has not changed. Whether it's a software or hardware issue, there should be some indication.


I have two panic logs. Panics occurred at:


  • new_vnode vfs_subr.c:3717. "free vnode still referenced".
  • pmap_pv_remove: pmap_internal.h:813. "pv not on hash"


This looks like memory corruption. If it's a kernel software issue, I have no idea where to start. If it's a defective memory issue, is there an on-board test to diagnose this?


Any assistance would be appreciated. Thanks.

If you have more than one user account, you must be logged in as an administrator to carry out these instructions.

Launch the Console application in any of the following ways:


☞ Enter the first few letters of its name into a Spotlight search. Select it in the results (it should be at the top.)


☞ In the Finder, select Go Utilities from the menu bar, or press the key combination shift-command-U. The application is in the folder that opens.


☞ Open LaunchPad. Click Utilities, then Console in the icon grid.

Select the most recent panic log under the heading System Diagnostic Reports on the left. If you don't see that heading, select


View ▹ Show Log List

from the menu bar. Post the entire contents of the panic report — the text, please, not a screenshot. That panic was not caused by third-party software. If the problem is recurrent, the possibilities are:


  1. A stale or corrupt kernel cache
  2. A damaged OS X installation
  3. A fault in a peripheral device, if any
  4. Corrupt non-volatile memory (NVRAM)
  5. An internal hardware fault (including incompatible memory)
  6. An obscure bug in OS X


You may already have ruled out some of these.

Rule out #1 by booting in safe mode and then rebooting as usual. Note: If FileVault is enabled on some models, or if a firmware password is set, or if the boot volume is a software RAID, you can’t do this. Post for further instructions.

You can rule out #2 and #3 by reinstalling the OS and testing with non-essential peripherals disconnected and aftermarket expansion cards removed, if applicable. Sometimes a clean reinstallation may solve a problem that isn't solved by reinstalling in place.


Corrupt NVRAM, which rarely causes panics, can be ruled out by resetting it.


If your model has user-replaceable memory, and you've upgraded the memory modules, reinstall the original memory and see whether there's any improvement. Be careful not to touch the gold contacts. Clean them with a mild solvent such as rubbing alcohol. Aftermarket memory must exactly match the technical specifications for your model.


The Apple Hardware Test or Apple Diagnostics, though generally unreliable, will sometimes detect a fault. A negative test can't be depended on. Run the extended version of the test.


In the category of obscure bugs, reports suggest that FileVault may trigger kernel traps under some unknown conditions. Most, though not all, of these reports seem to involve booting from an aftermarket SSD. If those conditions apply to you, try deactivating FileVault.

Connecting more than one display is another reported trigger for OS X bugs.

If your system is not fully up to date, running Software Update might get you a bug fix.


In rare cases, a malformed network packet from a defective router or other network device can cause panics. Such packets could also be sent deliberately by a skillful attacker. This possibility is something to consider if you run a public server that might be the target of such an attack.


If none of the above applies, make a "Genius" appointment at an Apple Store to have the machine tested. You may have to leave it there for several days. There isn't much point in doing this unless you can reproduce the panic, or if you can't, it happens often enough that it's likely to be repeated at the store. Otherwise you may be told that nothing is wrong.


Print the first page of the panic report and bring it with you.


Back up all data on the internal drive(s) before you hand over your computer to anyone. If privacy is a concern, erase the data partition(s) with the option to write zeros* (do this only if you know how to restore, and you have at least two independent backups.) Don’t erase the recovery partition, if present.

I'm looking at the Apple Hardware Test option. This is a late 2009 27-inch iMac. I purchased Lion and installed it directly. There is a recovery volume.


When I hold down the D key on boot, it does not enter AHT. It simply starts normally. Other keys such as Option work as expected. Is the AHT not on the recovery volume? How might I determine this or install it?

This is unusual and is likely to indicate a memory failure, logic board failure, or both.


RunApple Hardware Test.


If the Mac shipped with discs, insert the disc bearing the label "AHT Version" or "Apple Hardware Test" and hold the d key while it starts. Run the extended tests, though a failure is the only result that should be considered conclusive from using AHT to diagnose memory problems.


You must remove the firmware password, if one has been set, before you can use AHT.


Rember, a free GUI front end for memtest and may be useful to diagnose suspected memory failures that pass AHT's tests. However, it can only test memory that had not already been allocated to other processes.

The rescue partition contains Lion-specific code, so if the AHT was on the disk previously, then presumably it has been overwritten. Should the Lion USB stick contain a copy of AHT, or is it model-specific? Can a replacement be obtained?


I am running Rember now, recognizing that it can only operate on memory as it is made available by the kernel.

It's merely unusual. The memory might be perfectly OK. If the Mac's originally installed memory was removed for an upgrade, replace the original RAM (if you still have it) and determine if the kernel panics subside.


Your iMac would have shipped with discs since it predated Lion. You can obtain a replacement set from Apple. They will require your Mac's serial number.


You should have those discs for a number of reasons, but I wouldn't concern yourself with AHT. Unless it finds specific errors it will conclude "no trouble found", but failure for it to identify a specific fault does not mean one does not exist.

If you decide to purchase memory to rule yours out as a potential problem, purchase it from a vendor known to supply RAM that meets Apple's stringent specifications. The only two vendors with which I have personal experience and can recommend are Crucial and OWC / MacSales. Complaints regarding memory sold by other vendors are common on this site.


Replacing it would be easier to justify if AHT or similar test utility reported a failure, so unless it has, replacing it amounts to guesswork. From the information you have, and lacking the time-consuming and exhaustive hardware diagnostics available only from Apple or AASP, there is nothing else to recommend.
