iPhone 5s headphones won't initially work when making a phone call. But if I unplug it and plug it back the call audio can be heard.

Very possible there is some dirt or debris in the headphone port. Try cleaning that out with a small toothpick or straight pin. Make sure also, when you plug the headphones in that you completely seat the jack into the port. Even the most minute space when plugging in can cause it to not connect correctly.
Tried it on two phones and two different headphones and saw the same thing. Is there a setting that defaults to phone speaker/headphones?

No, there is a sensor in the headphone port to recognize that headphones are plugged in.When you receive and initiate a phone call with the headphones, it will pause the music in iTunes. There is no other setting for the headphones.
I can play music properly through the headphones. Pause it. Dial phone number and only head the audio through the external phone speakers. If i unplug the headphones, plug them back in, even while the same call is ringin, I can hear the audio through the headphones.
It may be due to your manually pausing music before you initiate the call. I'm not sure as I do not use the headphones that often to make calls. I have a Bluetooth headset when away from my vehicle and I have a Bluetooth setup in my vehicle for calls and other audio.
