Test this behavior in another user account. The guest account is fine.

Do you have any third party 'hidden feature/tweaking' applications installed which you use to modify preferences in the dashboard [ie onyx, which is now deprecated in mavericks]? If so, uninstall them according to the developers' instructions.
From the Finder Menu Bar, Go.. > Go to Folder ... > ~/Library/Preferences/
Copy the bolded path. A finder menu will pop up with property list files for your account. Delete the following:
com.apple.dashboard.client.plist
com.apple.dashboard.plist
com.apple.dock.extra.plist
com.apple.dock.plist
com.apple.desktop.plist
Reboot your machine and see if the problem still occurs.

don't have any apps which can modify dashboard or launchpad as well
Open Terminal. Copy and Paste the following bolded command. Enter it. Log out of your account and log back in. This will fix your problem. You can subsequently trash the file moved to your desktop.
mv ~/L*/Application\ Support/Dock/*.db ~/Desktop; killall Dock; exit

Thanks for the tip - fixed the initial (and hopefully only!) occurence of this on my system. Will be watching for a recurrente
Deleting Launchpad db is not a good idea: you'll lost the icons grouping in Launcher. If you have a lot of programms, it will be a headache to bring it back.
Just use Windows method - reboot, and you'll see your icons again. Until next time.
I've tried the method listed in response #4, where you go to preferences and delete certain files. It didn't work, it only removed my icons from the dock and set it back to default. The problem is, I can't open Terminal.. Like many of my icons in laucnhpad, Terminal is missing its icon too, and I can't open these programs. PDF-viewer, iBooks, Maps and several other programs can launch, but I can't interact in the program. What to do?
Hi, this worked for me -
First, free up enough disk space so that you no longer get disk-full warnings. Then, in the Finder, select Go > Go to Folder... from the menu bar, enter the following in the text box that opens, and press return:
~/Library/Application Support/Dock
Move the contents of the folder that opens to the Dock and close the folder. Log out, log back in, and test.
Okay, so that didn't work for me, and now I am missing all my icons in the Dock as well....
Killing the database and regenerating it only works temporarily as a few days later the problem reappears for me. As another person said, you lose your organization. I think this is a bug that Apple will need to fix ultimately. In the meantime, searching around for a solution I found this:
It's an app to help with launchpad and has a demo-mode. Simply clicking "Regnerate" in the app after starting it the first time fixed the problem for me -- however I don't know if this would do the same as before and lose organization. Since the app seems to be designed for organization, perhaps it keeps a separate database and refreshes the launchpad database. I'm going to check it out but thought I'd share it for others that might be interested in looking into a temporary fix as well.
