Apple changed the audio API in iOS 6, things just haven't been the same since.
Google Voice voicemail (america)
Planet 3 Austria music
Those are the three for me that don't have working audio on the external speaker but work fine with headphones.

I HAD THE SAME PROBLEM AND IT WAS FIXED.
Go to settings>icloud>storage and back up>back up
After download hold the devices home and power button for 15 seconds. itunes will detect an ipod in recovery mode. hold the option key and click restore.

I am willing try out your solution, but its not clear to me.
What are the steps?
1ste - Make a backup "settings>icloud>storage and back up>back up"
2 - download the software
3 - connect with iTunes ?
4 - Re-boot (hold the home&power buttons for 15 seconds)
5 - iTunes will detect my iPhone in recovery mode?
6 - and last now hold the option key and click restore
Is this the right way?

Robert, you were probably repotted because of promoting the last-ditch effort and not everyone makes good backups of their phones.
You have to tell everything if you want to suggest a solution and that is advising the user to back up before doing your method.
It's akin to saying Format C:/ to fix your problems, it's a drop-kill method, plus it can be seen as "trolling" we try everything before restoring to starting over with a new phone.
