No.. it sits on top of my desk the entire time. I realize its not indestructible, but it should be able to hold up to general handling.
I've found they do. I've never had any of the original iPhone cables or the lightning cables (I have about half a dozen) go bad. Even the one I slammed in my desk drawer is still working, though I expect it to die sooner rather than later.
From the look of the cable, it looks like you've been unplugging by pulling on the cable rather than on the connector shell. If that's the case, such a failure is not unexpected, as the repeated stresses as well as oil and other contaminants from your fingers can degrade the plastic over time. You should always unplug by pulling the hard plastic connector shell, not the cable. Over time the drying out of the plastic and regular flexing can cause such failures even if you're careful.
You can probably wrap the break with electrical tape and get a bit more use out of the cable - I did that with my three-year-old cable that similarly had an insulation fracture - but eventually you'll probably need to buy a new cable.
