Probably, because your iPhone has no idea what your iPad is downloading, its just providing a bridge to its internet connection.
So whatever the iPad downloads is not affected by the Phone settings.
That was my first thought too - but the iPad does realize that it's connected to a personal hotspot, rather than a regular wireless network, so it might be that the iPad would elect not to download the update. I'm not sure.
Yes, the iPad will download app updates over your iPhone's cellular data. I just tested it. I was tethered to my iPhone using Bluetooth, which I figured to make it even less likely to do so, but as soon as I turned on auto updates it started updating apps.
It's not obvious at first either. You can't see any progress bars. You will just notice the number of pending updates gradually decreasing, and in the App Store "Updates" section you will notice apps appear as updated on this date.

Seems pretty obvious to me that more often than not, if you're using a personal hotspot, you don't want to use data unecasarily.
Turn Off Auto Updates. That way you need to manually initate the Updates before they'll start downloading.
