Oh man, Josh Himself, you TOTALLY rocked it! Only caveat... the preference window must be closed and you have to add your password when asked. But that totally fixed it.

Just uncheck the box.
I asked how to remove items from the list. If you don't know, that's ok. If anyone DOES know, please contribute a positive comment. Thanks for faking the time to respond.
Is the goal to deny access to the app in question? If so, uncheck the box.
There is no provided method for removing an item from the list, probably because that makes it easier to later grant access to an app that had previously requested access.
The goal is to remove apps that don't belong there, such as obsolete and deleted apps. There are suggestions on how to do this via Terminal in 10.7 and 10.8 here.
The only way to do it is to edit the plist as you linked, but seems like a large waste of time.
Thanks guys. I always try to keep obsolete stuff off my machines whenever possible, especially when the OS says the list is for apps to control the machine! Just seems like a sensible practice to me.
It's just a text file. It isn't keeping your app alive or anything.
Well some apps are still on the Mac and I wanted to make sure they don't get checked inadvertently. THanks again for the reassuring posts.
I would also like to do this. My use case is slightly different: I'm writing some documentation, and want to take screenshots of the full process. Once apps have been added to Security & Privacy > Accessibility, it's hard to take "clean" screenshots that illustrate the specific instructions I'm trying to write.
(I know that Apple has knowledgebase articles on the topic. Those are general, and very helpful to me, but not to what I'm documenting. I'm writing instructions that are specific, to a particular app and process flow, and it's important that the screenshots be specific, too, not generic.)
Erase your drive, Clean install your system and just reinstall the Apps you want.

You have to find the plist file that stores that info (or possibly the sqlite3 database), then delete or edit it.
sudo sqlite3 /Library/Application\ Support/com.apple.TCC/Tcc.db 'delete from access where client like "%appnamehere%"'
Replace "appnamehere" with the name of the app as it appears in the Accessibility list (keep the % signs).
