the Notes application thinks that (in addition to my icloud account) there is an exchange account hooked to it (even though notes is not checked in the accounts systems preferances pane for that account). The app periodically asks for the password for this exchange account, but doesn't connect even when the correct password is entered.
How do I delete this phantom account from Notes?

I have this same problem, too. I had some issues with Mavericks and my exchange account when I updated to Mavericks. But, there are two phantom exchange accounts in my Notes account left. I have gone into Sys Prefs and Notes are not checked. I have found no way to delete them. The phantom accounts do not appear on any other devices. I cannot find any prefs in my library. My notes app also crashes after about 5 minutes of being opened. How can I delete these accounts?
Does anyone have a solution to this? Short of re-installing Mountain Lion, or removing the OS X user account I can't get rid of this.
This worked for me... IN Notes I went to Notes--> Accounts --> and it showed the accounts that were checked. I had to enter my password and "trust" a server and then I was able to click cancel and the uncheck that account. I then closed NOtes and when I reopened Notes, the "Phantom" account was gone. Hope this helps.
I've done a CLEAN install of Mavericks (due to some unreleated issues). I thought that would certainly clear this phantom account, but the same notes account still shows up -- even though Notes was never checked in the Accounts tab. Must be some Exchange -Mavericks interaction.
Stand_44, your method worked for my Notes app in Mavericks, thanks! Now the problem I'm having is with the Notes app on my iPhone. The "phantom" account is still showing up there, even though it's not visible in the Mavericks app or on iCloud.com. I'm very confused, and the Notes settings on the iPhone don't give an option to remove accounts. I'm scared to mess with the iCloud settings on my iPhone, since I don't want to lose the notes synced with my iCloud email address (I already had problems with losing saved notes when I created the iCloud email address in the first place so I'm a little skittish). Anyone having this same problem?
OK -- finally sorted this out!
1) unchecked Notes on all accounts (System Preferences/Accounts)
2) turned off Notes in Systems Preferences/iCloud
3) moved everything in ~/Library/Containers/com.apple.Notes/ to the trash
4) restarted --
5) turned on icloud notes again
6) restarted Notes.app
I now have ONLY my iCloud notes account listed!
