Disk0s2 I/O Error

I have been looking over this discussion board for answers regarding how to fix this error, and there are some excellent responses. The question I have is how does this problem even start? How can I prevent from having to replace another hard drive in my MBP?

I am getting this error again, but this will be the third time I've replaced this harddrive. Is replacing it on my own the issue?

I have to bring this discussion back from the dead. This issue is getting rediculous. Now, the computer doesn't even need to restart to "lose" the hard drive. I have a USB thumbdrive that has OS 10.8.2 and 10.9 installation disks, but I can't even boot into recovery mode.

When I boot with the 10.9 installation disk, sometimes my hard drive isn't even visable. I have to boot with the 10.8.2 installation disk just to run disk utlity so I can get my macbook to boot. Someone out there has to know something.
