Mid-2010 13" Macbook Pro on Mavericks - kernal panics

I'm not very hardware literate, so the error reports mean almost nothing to me. I've been having problems with my mid-2010 13" Macbook since Mountain Lion - which I realize is a known issue. My laptop will run fine for a time (usually a couple hours if I'm not doing much, thought it can be much shorter) then the screen either fuzzes or goes a solid color, and it crashes. After the crash it automatically restarts with the "restarted because of an issue" message. Then that restart freezes and I'm forced to manually restart my computer. After the second restart my computer gives me an error report (at end of post).


The reason that I'm posting this is that I've had to clean my computer out, and at this point I'm running almost entirely first-party. I've followed a few dianostic posts (including Apple's) and havent had much luck. A week ago I did an SMC reset and it greatly improved performance and alleviated many issues I had been having. I'm at a loss at this point of what to do, and would love some help.
