An unread email arrived to my iPhone where instead of showing the usual blue dot for unread, it shows as a smaller blue dot with an orange circle around it. I've never seen this before. What does it mean?

That indicates the email is flagged. The icon for a flag can be changed by opening the Settings App > Mail,Contacts,Calendars > Flag Style. You have the option of the "dot" you see now, or a flag shape.

Also, it appears as I described on my iPhone, but as just a blue dot on my iPad.

Two different functions. Unread is not a flag. Blue dot is Unread. Orange dot or orange flag (depending on the settings) is a flag and statys set until you unflag it, regardless if you have read it or not.
