how can i undo forget this device for bluetooth headset with iphone 4?
I have plantronics M50 bluetooth headset and by mistake i hit the forget this device. now i am unable to connect to bluetooth headset with my iPhone 4

Try cycling power on the Plantronics headset at about the same time as you turn the Bluetooth on on your iPhone.
Doing this will set both your iPhone and the Plantronics into discovery mode at the same time, so they can find each other.

when you check your bluetooth headset on your iphone the Charge should be very low
for me it worked
I did this with my Plantronics earpiece. Turning it on, and holding the activation button for several seconds made it reappear on my iPhone Bluetooth preference screen.

One other thing. I have the iPhone 6s Plus. After I updated my Bose Bluetooth Headset Series 2 I wasn't able to pair. According to Bose you're supposed to be able to just "Power ON" your device and it becomes discoverable. Sometimes your iPhone 6s Plus STILL will not discover your Bose Bluetooth Headset Series 2. To get it to re-discover your headset do the following:
1. HOLD DOWN the CALL BUTTON for 5 seconds while powering on your headset.
2. Give the iPhone 6s Plus about 6 or 7 SECONDS.
Your iPhone 6s Plus will now discover your Bose Bluetooth Headset Series 2.
