I'm about to buy a refurbished Macbook but cannot see any explanation of exactly what has been refurbished? How can I find out this detail?

Or is there no need to worry or know this as it will be covered in the warranty anyway?
Just like buying a car, you'd like to know what went wrong in the first place in case it's a common default and could potentially happen again.
Apple does not tell you which part was repaired, but any problem you have is covered by warranty, so it does not mind
Does the macbook air come with a new outer shell? I know it did back in 2012 but I think I read on apple's website that the refurbished model might have some scratches on it.
depends on unit to unit, generally no.
Nobody should worry about a few minor micro scratches on an alloy chassis. 😊
Because they are individually tested and use the latest known parts, firmware and other changes, in some ways they could be said to be "better than new." I know that has been my experience. And immaculate. Good value.
