I have and iPhone 5 and the camera on my phone is loose, rattles, and almost falling out. I have already replaced my phone months ago through an apple store because my lock button jammed. Would apple replace/fix my phone due to the camera being loose for free?

Yes, if still under warranty and determined not to be caused by user damage.
I dont know if its under warranty. I don't remember when I bought the phone but it shouldn't be user damage because I've dropped it once or twice. There aren't any marks on the phone. It looks new.
