The extension cable to the keyboard totally "fixed" it. Thanks for posting that!!!
I hope Apple gets these bugs under control.

I have MacBook Pro 2016 and Dell U2515H and connect Apple keyboard to monitor.
When I unplug and plug monitor from MacBook (USB concentrator), Apple keyboard doesn't work, and I need to wait ~10min and plug it again.
But today I tried with extension cable and it fixed this issue with plug/unplug keyboard
And seems like a problem in OS X because with extension cable it works perfect and this problem reproducible in other MacBooks I tried.

Connect the keyboard directly to the usb port on your mac.
That's it.
Don't run your keyboard through a hub. I was running mine through the hub that is built in to my monitor. It's something to do with the hub not drawing enough power to keep the port active. I don't really get it, but I'm typing right now so it works.
For a $3500 computer you wouldn't think this would be an issue, but here we are.
